How to register as a new patient at Leith Surgery - please read through the steps
1. Check if you are eligible
New permanent patient registrations:
To register as a new permanent patient you must be living permanently within our practice area. This means having a fixed address which you plan to live at for at least three months. Check your address in our catchment area map
You cannot register at Leith Surgery if you are already registered with another local practice and are eligible to remain on their list of patients
Children should usually be registered at the same practice as a parent or main guardian
Temporary registrations:
If you are living here for less than three months you should register as a temporary patient. Do not register as a temporary patient if you are a student here for more than three months - instead, register permanently.
2. Fill out the registration forms and visit reception with proof of identity and address
Permanent patient registration:
If you have confirmed that you are eligible to register, please complete our online application process
You must then visit reception in person with ID and proof of address within 10 days of completing the online application process. Please visit between 10.30-12.30 or 13.30-16.00.
You must visit reception within ten days to activate your application otherwise your data will be deleted from our systems.
Temporary patient registration:
please complete the temporary registration form, available from reception in paper format only.
3. Completing your registration
We complete most new registrations for eligible patients within ten days
For patients transferring their medical care from another NHS GP practice in the UK, a request will be made to transfer your existing medical records to us, but please be aware that there can be extremely long delays beyond our control, so if you need prescriptions in the meantime you will need to give us a copy of you most recent prescription(s) and copies of your medical notes/letters.
Patients transferring their primary medical care from abroad are responsible for giving us any existing medical documents which we need and, where applicable, these must have been professionally translated into English language.
Changing your details
Keeping your contact details up to date is extremely important to allow the surgery and NHS Scotland to be able to reach you about your healthcare. If you have changed your phone number or address since you registered, please complete the online change of address form. For security purposes, we need you to use an email address already on file - if this is not possible, you can complete a paper form at reception.
If you change your address and move out of our practice area, you will need to change your GP surgery; you will have 30 days to do so. It is important to update your address and your GP surgery when you move away so that you can continue to access urgent and routine healthcare in the future and receive important and confidential letters at your correct address.
